School, Camp, and Sports Physicals
Northwest Pediatrics is glad to support children’s healthy lifestyles by providing forms for school, camp, and sports physicals.
To receive one of these forms, children must be up-to-date on their check-ups (having had one in the past 12 months). Please download the medical forms below. When you have filled the parents’ section in and brought or faxed (336-605-0930) them back to us, we will complete them within 48 hours. There is a $5 administrative fee for us to complete these forms.
For Family and Medical Leave (FMLA) forms, please provide the information that the form requires, and please allow a week for us to complete the forms. The administrative cost for FMLA forms is $25.
